Incident Reporting and Investigations

CRA can provide workplace accident investigation, reporting, analysis, and litigation support.  Through the accident/injury investigation process, CRA can determine the factors that may have contributed to an accident and define the contributory deficiencies.  Training programs and engineering controls can be developed to ensure that future incidents do not occur.  Record keeping services and assistance with training personnel are also services that CRA can provide.

CRA has developed a web-based Environmental, Health, and Safety (EHS) Incident Reporting system to improve the reporting and tracking process to compliment our clients' EHS programs. The application provides:

  • Detailed incident forms for both environmental and safety occurrences completed through a web browser
  • Auto-generated incident number used for tracking and querying
  • Records treatment and lost workdays
  • Area for detailing cause and preventative actions (with email notifications)
  • Area for detailing any associated vehicle and property damage
  • Tracking of incident until closed (approval required by selected levels of management) and then archived
  • Internal reports: recordables, monthly indices, details on specific incidents, action items
  • External reports: OSHA reports (300, 300A, 301), WSIB (Ontario)
  • Query data based on various criteria
